Patients of Northland Community Health Center (NCHC) are encouraged to apply for discounted health care services through our Sliding Fee Discount Program (SFS).  The process is simple and only takes a few minutes.  The following programs may be available to you: 

Program		Potential Discount
Medical Discounts:            The SFS Program applies a discount to the cost of the  services, allowing individual's access to affordable, quality, primary health care needs.		¨       20 – 100% discount on services performed at NCHC by NCHC providers
Dental Voucher Program: If on the SFS program, patient's may receive, if eligible, a dental voucher (discount) towards a visit to one of NCHC's contracted dentists.		¨       $100-$140 voucher for dental services with a NCHC contracted dentist
340B Drug Program: Allows our patients, if eligible, to receive a discount on formulary medications thru our contracted pharmacies.		¨       Discounts on formulary prescriptions at participating NCHC pharmacies
Behavioral Health Services: This program is provided through arrangements with West Central Human Services, McLean Family Resource Center and Rural Mental Health Consortium.  Services include: Information & referrals, crisis intervention and counseling.		¨       20 – 100% discount on appointments
